Influenza vaccination point subsidies

Flow

STEP 1	Confirm the “eligible persons” and “subsidy period.”
STEP 2	Confirm the “Application Manual” and “FAQs.”
STEP 3	Make an appointment to receive the influenza vaccination.
STEP 4	If you have not yet registered for Pep Up, please do so. POINT！　It is only possible to apply for FR Health Insurance Organization influenza vaccination point subsidies through Pep Up. Make sure to prepare so you can apply as soon as possible after getting vaccinated.
STEP 5	Get the influenza vaccination you made an appointment for. POINT！　Make sure to get your receipt.
STEP 6	Use the various application features of Pep Up to apply for the influenza vaccination point subsidy. POINT！　If your application is correctly received, we will send a “Decision Completed email.” If there is an error, you will be notified within Pep Up. If you receive such a notification, please apply again as soon as possible. Reviewing the application details will take around a month.
STEP 7	If there are no problems with the application, points will be awarded to those who apply for points after the 20th of the month following the month of application. POINT！　Step 6 of the application process must be completed by March 15th of the relevant year.

Eligible persons

Persons who satisfy one of the conditions below and are eligible members of the FR Health Insurance Organization on both the vaccination date and point award date:

• Dependents enrolled in the FR Health Insurance Organization
• Voluntarily and Continuously Insured Persons

• * Persons who are not eligible as described above cannot apply.
• * Employees of the Fast Retailing Group who are enrolled in the FR Health Insurance Organization are not eligible.
• * Employees interested in subsidies should contact their companies for details.
• * If you gained FR Health Insurance Organization eligibility in January 2026 or later and would like a subsidy, please inquire with us by using the following form:

Vaccination period: October 1, 2025, to February 28, 2026
Application period: October 1, 2025, to March 15, 2026

• * It is necessary to register for Pep Up to use it. If you have not registered, please do so as soon as possible.
• * After applying, if there is an error, etc., you will be notified within Pep Up. Please apply again by March 31. Once the deadline passes, it is no longer possible to apply to have points awarded.

• * Pep Points are not directly awarded to vaccinated persons but rather to the person who completed the Pep Up application procedure.
  Example) If the applicant is an insured person and the vaccinated person is their spouse, the Pep Points are awarded to the insured person who applied.
• * If eligibility has been lost when the Pep Points are supposed to be awarded, no points will be awarded.

• Dependents: Spouses and dependents aged 40 or older may only apply for their own vaccination (including Voluntarily and Continuously dependents)
• Voluntarily and Continuously Insured Person: May apply for themselves and dependents enrolled in FR Health Insurance Organization
• Employees enrolled in FR Health Insurance Organization (Insured persons) : May apply for only dependents enrolled in FR Health Insurance Organization

• * If the applicant has lost their FR Health Insurance Organization eligibility as of the point award date, no points will be awarded.

Required documents

A receipt that includes the six items below is required.
Receipts will not be accepted unless they meet all of these requirements.

• Name of vaccination: “Influenza vaccination fee”
• Name of recipient (full name)
• Date of vaccination
• Vaccination fee (per recipient)
• Name of medical institution
• Medical institution receipt stamp (If the receipt does not state “Stamp required for validity,” a stamp is not required.)
