Appointment period : Monday, April 1, 2024 - Tuesday, December 31, 2024
Checkup period : Monday, April 15, 2024 - Friday, March 14, 2025

Family members: "spouses and dependents of age 40 or over"

Appointment period : Monday, April 1, 2024 - Wednesday, January 15, 2025
Checkup period : Monday, April 15, 2024 - Friday, January 31, 2025

Caution:

  • * Please schedule health checkups at least three weeks from the date you ask for an appointment.
  • * You cannot make an appointment or receive a health checkup on dates other than those listed above.

Appointment methods

Note that adjusting the schedule with the medical checkup facility, and other checkup procedures will take time, so please apply at least three weeks before your desired checkup date, and be sure to specify a checkup date.
In addition, some medical checkup facilities specify a period between when you return to Japan and your checkup date, so be sure to confirm the website of the medical checkup facility where you plan to make an appointment before specifying your checkup date.

Caution:

Members who do not have addresses in Japan

When making an appointment upon temporarily returning home, members who do not have addresses in Japan may use an FR Wellness Center address for the purposes below.

1.Receipt and storage of specimen containers
Specimen containers sent from medical checkup facilities can be received and stored at a Wellness Center. Please have either the Ariake or Roppongi office receive these containers.
Please note that we cannot send specimen containers to personal residences or overseas posts.

2.Receipt and sending of checkup results
Results sent from medical checkup facilities can be received at a Wellness Center, and they are then sent to the overseas office at the most recently registered address via regular overseas mail.

Receiving checkup results

If you registered a company address upon registering with the checkup appointment system "i-Wellness" to receive a checkup kit, etc., when you have your checkup, be sure to tell the person at the medical checkup facility help desk to change where your checkup results will be sent. (Change to an address in Japan where you can receive your results.)
