To those who receiving medical expense subsidies

Please notify us if you are receiving medical expense subsidies from municipalities

Our health insurance organization automatically provides insurance benefits to those who individually pay over a certain amount of medical care costs, but there are also "medical expense subsidy systems" of local governments (municipalities as well as prefectural) that assist infants (children), single-parent families, persons with disabilities, etc. with the payment of medical care costs by subsidizing copayments. Those who are receiving these medical expense subsidies from local governments are not eligible to receive insurance benefits from our health insurance organization because these would result in double benefits. (*)

However, the details of medical expense subsidy systems often differ depending on local governments, and due to a lack of relevant information from medical institutions, it is extremely difficult for our health insurance organization to understand the situation unless relevant persons notify us of the details.

If you receive benefits both as a result of the medical expense subsidies of local governments and from our health insurance organization, we might have you return the benefits at a later date, so be sure to notify us in such cases.

Note that—in the case of children who are 15 years of age or younger—the relevant local governments generally provide medical expense subsidies, so the automatic provision of benefits by our health insurance organization is stopped.
If you have paid high medical care costs for such a child, please contact us.

* Double benefits: Here, this term refers to cases where benefits are received from both our health insurance organization and local governments.

Applies to:

People or their dependents who are able to receive medical expense subsidies from local governments
(Medical care for physically and mentally disabled persons, medical care for single parent households, medical care for expectant and nursing mothers, etc)

  • * Please notify us in the case of a child who remains eligible for a medical expense subsidy after they pass the school age (the end of the fiscal year when they turn 15).
  • * Please notify us if you are exempt from individual payment or receive reductions for any other cases.
  • * If necessary, our health insurance organization might make an inquiry.
  • * The names and subsidy details differ for each local government. For details, please check with the municipal office.

For those who have completed receiving their medical expense subsidy

Please submit a notification of termination when you become unable to receive subsidies during the period of the medical expense subsidy that has already been applied for.

  • If the subsidy has ended due to your income category, etc.
  • If the subsidy has ended due to a change in your place of residence
  • If the subsidy has ended because you no longer satisfy the subsidy requirements

Caution:

  1. Notification is not necessary for those who are not receiving medical expense subsidies.
  2. Please submit notification forms for each person in the event there are multiple people who are eligible in one household.
  3. If the concerned person is residing overseas, please submit when returning to Japan.
  4. We may request those who are eligible for medical expense subsidies that have not notified us to once return the paid insurance benefit on a later date. We kindly ask for your understanding.
