⇒ Please submit a Notification of Change of Address to your company (in charge of social insurance).
- * Note that the addresses of dependents are not automatically changed when the addresses of insured persons (enrollees) are.
If it is necessary to change the address of both an insured person and a dependent, please do (1)/(2) at the same time.
