Notes:	If you or the counterparty has voluntary insurance associated with a settlement agency service, you can receive support from the responsible casualty insurance company for preparing and submitting documents, including the Notification of Injury or Sickness due to a Third-party Act and the Accident Report. Please check with the insurer with whom you contracted your policy for more information. ** This support is available only for traffic accidents away from the job for which you used health insurance.

How to receive a traffic accident certificate

• Apply to receive a traffic accident certificate by submitting the prescribed postal transfer form to the “Safe Driving Center Bureau” in the prefecture where the accident occurred.
• Postal transfer forms are available at any central or local police station, police substation, accident insurance company, or JA Co-op.
• After making the application, the Driving Center Bureau will send the certificate to your home address (or other address as requested).
