Acupuncture, moxibustion, massage, and shiatsu are covered by health insurance benefits (for medical care expenses) if certain conditions are met.

Required documents:	Application Form for Medical Care Expenses (for Acupuncture or Moxibustion) Example
	Application Form for Medical Care Expenses (for Massage, or Shiatsu) Example
	[Documents to attach] Letter of approval from an insurance doctor Receipt (original) showing the name of the person who received treatment Report on treatment (copy) (if one is provided by the practitioner)
Submit to:	FR Health Insurance Organization
Deadline:	As soon as possible
Applies to:	Insured persons and dependents meeting the conditions for payment of medical care expenses and have been examined and received letters of approval from insurance doctors
Inquiries to:	FR Health Insurance Organization
Notes:	If continuing treatment is deemed necessary after six months (one month for manual correction of deformities), a second letter of approval from an insurance doctor must be obtained.
